The purpose of this form is to assist you in preparing your request to the committee. You may check the appropriate boxes and attach supporting documentation. The written request should then be forwarded to the Architectural Committee. 

The coordinator of the Architectural Committee will process it and return a signed approval form back to the requestor within ten (10) days.
